How to Use Your Program Card
If eligible for the CINQAIR® Cost Support Program, you can use your Program Card with your treatment provider to help pay your out-of-pocket costs for treatment with CINQAIR® (reslizumab) Injection.
After you receive your treatment, your treatment provider will submit a claim to your insurance plan. Your treatment provider may or may not collect your co-pay, based on your insurance coverage.

You will receive an Explanation of Benefits (EOB) statement from your insurance plan. It will show your out-of-pocket costs for CINQAIR® and its administration. This will determine the amount the Cost Support Program will issue to your card.

You or your treatment provider can fax or mail your EOB to the CINQAIR Cost Support Program. No other forms are needed.

Avoid delays in processing program benefits by coordinating with your treatment provider to send one EOB after each infusion with CINQAIR®.

Once funds have been issued to your Program Card, you can use it with your treatment provider to pay your out-of-pocket costs for treatment with CINQAIR.
*The CINQAIR Cost Support Program helps commercially insured individuals in the United States (including the United States territories) who are prescribed CINQAIR pay for their eligible out-of-pocket costs for the drug and for administration of the drug, including co-pays, co-insurance, and deductibles.
Persons residing in a state that does NOT allow financial assistance for the administration of the drug may ONLY be eligible to receive out-of-pocket cost support for the drug.
The Program does not cover the costs of physician office visits or evaluations, blood work or other testing, or transportation. Eligible individuals must have commercial insurance for CINQAIR. Uninsured and cash-paying patients are NOT eligible for this Program, nor are individuals with commercial insurance coverage that does not provide coverage for CINQAIR.

If you are a Medicare beneficiary in need of co-pay assistance, contact these non-profit organizations for details that may be able to help you with the cost of your medication in the form of a grant.* Please note these organizations are independent of Teva and establish their own eligibility requirements. Teva does not endorse any particular organization, nor make any representation of who may qualify.

How the process works:
- Contact the 501(c)(3) organization to apply for a grant.
- Application requirements vary per 501(c)(3) organization – check with each organization.
- When the approval letter is received, provide that information to your Site of Care. Take the approval letter with you to every appointment at your Site of Care.
- Note that the billing process between Medicare, the 501(c)(3) organization and your Site of Care can be a lengthy process.
- Grants are generally approved by foundations for a period of 12 months
- The 501(c)(3) organization that provided your grant generally will reach out to you approximately 2-3 months prior to funds expiring regarding the reapplication process. If you do not hear from the 501(c)(3) organization regarding reapplication, you may reach out to them to inquire.
- You must reapply to receive additional funding. Once an approval letter is received, go back to Step 3 and repeat the process.
